Transitional Memory Care
When you need more care than is offered in Assisted Living, but less care than you get in Memory Care, our Transitional Memory Care Program—called The Residence Club™—may be just what you need.
Our community also includes full Memory Care for those with dementia or Alzheimer’s Disease. Called The Path to the Present® Resident Directed Program, our services keep your loved one engaged and safe 24 hours a day, seven days a week.
When you need a short-term place to stay during an illness, after a surgery, during rehabilitation or when a caretaker is away, come to us. You get a comfortable, fully furnished apartment with hotel luxuries like concierge services.

At Green Oaks, we do not offer skilled nursing. Nursing homes—also called skilled nursing facilities—offer a higher level of medical care than you’ll find at Green Oaks. With their 24-hour nursing staffs, they’re for people who don’t need a hospital but can’t be cared for at home. If you don’t need that kind of care, our independent or assisted living communities might be just what you’re looking for.
